An inquest’s heard one of the victims of the Lindt cafe siege in Sydney last month was killed by fragments of a bullet fired by police.

The inquest confirmed he was responsible for killing the other victim – cafe manager Tori Johnson – prompting officers to storm the building.

He died himself moments later.
