Older people face a high risk of receiving incorrect medicine after a hospital admission.
A new survey from the Royal College of Surgeons says 72% of older people are at risk. Dr. Frank Moriarty, from the RCSI, says there are various reasons why incorrect medicine may be given to patients. He says one factor could be the chain of command in hospitals.
